Multiple Teams · Physicals and Parent Permission Forms for 2020-2021 School Year

Are you planning on participating in a MSHSAA sponsored sport at LHWHS during the 2020-2021 school year?  If so, the Activities Office must have a current physical and parent permission on file.


The Missouri State High School Athletic Association has changed the physical exam requirements for MSHSAA sponsored athletics.  MSHSAA Bylaw 3.8.1 and 4.5.4 now allows a physical exam certified by a medical professional (MD/DO/ARNP/PA/Chiropractor) to be valid for a duration of two years (730 days) from date of issue (unless a physician specifically notes on the pre-participation exam that the physical is only valid for one year).

  • Example:  A physical that was received May 29, 2018 is now valid through May 28, 2020.   A physical that was received on August 2, 2018 is now valid through August 1, 2020.  Etc.

That being said, while the MSHSAA pre-participation physical exam form is now valid for a two year period from the date of the physical exam, all other pre-participation documents must still be completed/updated annually for every student athlete.  Students will not be permitted to participate without these documents on file in the Activities Office.
