LHWHS Activity and Athlete Registration for the 2026-2027 School Year As a reminder, the Ladue High School Activities Department has partnered with FinalForms, an online forms and data management system. FinalForms allows families to quickly and easily complete and sign athletic and activity participation forms. One of the biggest benefits is that your information is securely saved from season to season and year to year, so you won’t need to re-enter the same details each time. The system also pre-populates fields whenever possible, helping to save you time. Registration is now open for the 2026-2027 school year. All parents/guardians of students planning to participate in MSHSAA-sponsored athletics and/or activities are required to create a FinalForms account and register their student(s) if they have not already done so. This includes all high school athletics, as well as band, orchestra, choir, chess club, speech and debate, and scholar bowl. Please note that paper forms will no longer be accepted. In addition, all student-athletes must upload a current physical to FinalForms each year. If you uploaded a physical last year, you will still need to upload it again this year. However, you can simply download the document from your account, rename it, and re-upload it. You can review and update your information at any time. Forms must be signed once per year and again after any updates are made. If you have already created an account, there is no need to create a new one.
